I've visited the blogtalk radio site a few times and am confused as to how it works. I'd like to be a guest on someone's show, do an interview, talk about the upcoming book, my previous book, etc. Anybody have any ideas/suggestions? Thanks.

The way I got my interviews was to go to the blog associated with the radio show. There is usually a link on the show's site and sent them a message. I've had a lot of success with that.

A non profit that I work for recently began a blog talk radio show. We do it monthly, 30 min at a time, and it's free.  It seems to me that you just need to find the genre in Blog Talk Radio-land and contact them about being a guest. That's pretty much how it works for us; we are an adoption support group so we have people who have written books on the topic, as well as counselors, etc. discuss different adoption topics.  Hope that helps!
